I agree that it is always best to include an SASE, or is you are writing to someone in a foreign country an SAE with IRCs to cover postage. Some celebrities will pay, whereas some won't, but your chances of reply will be greatly increased if you do include one.
Photos are always optional, I tend to prefer the ones celebrities send, but sometimes if there is a photo I love I'll send that. There are a handful of stars who don't have their picture to send out, but not many. If you check the feedbacks in the database you'll be able to work out whether or not you need one.
For your letter: the main thing is to be polite, and originality is good too.
You can send a questionaire, - but bear in mind not all will fill them out - , don't make it too long though as they are busy people and it might put them off answering it.
